Hospice Conditions of Participation
In accordance with Hospice Conditions of Participation 418.52 and 418.104(a)(2), Lighthouse Hospice is required to:

– Obtain a signed acknowledgment from patients confirming receipt of this notice.

    This notice informs individuals about the availability of free language assistance services, as well as auxiliary aids and services, to ensure that information is provided in accessible formats. These services are offered at no cost to ensure clear communication and understanding for all patients and their families.
